Family Support is a non-entitlement publicly funded program designed to assist individuals living with an intellectual/developmental disability maintain a cohesive family unit, both living at home, and within the community setting.

Each individual approved for Family Support will receive an Individualized Family Support Plan (IFSP), which will identify the services and supports that are determined to best help the individual and family members, along with an allotted budget for these services and supports. Only those services and supports listed on the IFSP are eligible for Family Support funding.

Eligibility

​AADD provides Family Support through a state contract with the Department of Behavioral Health and Developmental Disabilities (DBHDD). In order to be eligible for services, individuals must be:

  • Three years of age or older
  • Diagnosed with an intellectual/developmental disability
  • Able to provide supporting documentation of intellectual/developmental disability diagnosis
  • A resident of Clayton, DeKalb, Fulton, Gwinnett or Rockdale county

Medical and Nutritional Supplies

AADD is excited to offer medical and nutritional supplies (e.g., incontinence supplies, chucks, food thickeners and so much more) through Professional Medical Fulfillment (PMF). This service offers direct-to-door  delivery of the name brands you know and love, ensuring families always have the supplies they need.

Requesting Funding for Services and Supports

To utilize allocated Family Support funds, participants must complete and submit a Family Support Request Form and supporting documentation (e.g., receipts, invoices, prescriptions, timesheets, etc.) to AADD’s Family Support Coordinator. AADD knows that families are busy and we are dedicated to helping families navigate their requests for services and supports in the easiest possible way – EMAIL! This allows families to make request anytime of day, when most convenient for them!

We also accept requests received via U.S. mail or fax.

Independent Contractors

AADD is dedicated to the individuals and families we serve. Our families select the vendors and contracts that meet their needs. Family-selected vendors and contractors must provide the required documentation before services and/or supports can be provided. Please note that AADD does not accept unsolicited applications for contractors.
